Information for Consumers

Consumers are entitled by law to receive treatment in a safe environment, free from abuse and neglect. Consumers are also protected by policies and procedures which ensure dignity and independence. Actions which violate these laws, policies or procedures can be reviewed by the Disability Services Ombudsman.

Consumers or consumer advocates who wish to file a report with the Disability Services Ombudsman may do so by completing the online form located below. Please complete the form as thoroughly as possible. Be sure to include your e-mail or telephone contact information.
